Athletics

Our athletic teams participate in the Diocese of Metuchen Catholic Youth Ministry leagues and other area invitational tournaments. We offer competitive teams in:

• Girls and Boys CYO Basketball
• Girls and Boys Cross Country
• Girls Cheerleading
• Junior Spartans Basketball
• Junior Spartans Wrestling

All student-athletes must have the below current medical forms on file before participating in any athletic program at Immaculate Conception School. Families are responsible for ensuring that all required forms remain up to date for each sport in which their child participates. Students will not be cleared to participate until the required documentation has been received.

Preparticipation Physical Evaluation Medical Eligibility Form: This form must be completed and returned to Immaculate Conception School before your child can be cleared to participate.

NJ Department of Education Health Forms: These additional forms may be downloaded if requested by your child’s physician. They are for use by families and medical providers and do not need to be returned to Immaculate Conception School. Please provide them to your physician as needed and retain a completed copy for your records.

All required medical forms should be sent to school with your child in a sealed envelope labeled: Attention: Mr. Peter Minsavage, Sports Physical Forms
